Becoming a Patient

Under Maryland law, only a state certified, licensed practitioner can decide if you qualify for medical cannabis. Once a physician has recommended cannabis, you must register as a qualified patient with the state to acquire your medical cannabis card. Any non-Maryland resident currently in Maryland for the purpose of receiving medical treatment is eligible to register as a patient and, during registration, will be required to provide the address of and a contact name for the Maryland medical facility where that patient is receiving treatment.

Here are the steps you need to take to become a qualified patient of Curaleaf MD:

  1. Register as a patient with MMCC
  2. Obtain a written certification from a provider registered with the Commission
  3. Visit a licensed Maryland dispensary
  4. Purchase ID Card (optional)

Qualifying Conditions

A patient can be certified to use medical cannabis in Maryland state if they suffer from one of the following:

  • Cachexia
  • Anorexia
  • Wasting syndrome
  • Severe pain
  • Severe nausea
  • Seizures
  • Severe or persistent muscle spasms
  • Glaucoma
  • Post-traumatic stress disorder
  • Chronic pain

Additional qualifying conditions are being considered and added to the program on an ongoing basis.

Patients must be Maryland residents at least 18 years of age; if you are under the age of 18, there are limited circumstances in which you may be qualified for medical cannabis use. Please consult a medical professional for more information.

Learn How to Certify Your Patients

In order to be able to issue written certifications for medical cannabis to patients for treatment of a qualifying medical condition, you must register as a “Certifying Provider” with the Maryland Medical Cannabis Commission.

To register as a provider, the application form must be completed and submitted in one session; you will not be able to save and complete the application at a later time.

  1. To complete the online registration form for provider, you will need:
    1. A valid, accessible email account which you are comfortable using in relation to your Maryland Medical Cannabis account. The Commission will use this email address as the principal means of communication with you regarding your account and its status.
    2. Your Maryland Board of Physicians License Number (available from the Maryland Board of Physicians)
    3. Your Controlled Dangerous Substances Registration Number (available from the Office of Controlled Substances Administration)
  2. Complete and submit an online application. Visit the MMCC Registry website, click “Create Account” under the Provider heading, then complete and submit the application form that displays.
  3. Verify your email address. As soon as you have completed and submitted the online
    application, a verification email will be sent to the email address you provided on your
    application. You must click the verification link in the email message to verify your email
    account. Without performing this step, your application is not complete.
  4. Once your email account has been verified, you will receive a follow up email confirming your registration with the Commission. Please retain the confirmation email for your records. It contains the Username you created during registration, which you will need to log into the Physician Registry, as well as your MMCC Physician ID Number.

Once your registration has been confirmed, you can log into the Provider Registry to access your account and issue written certifications to patients registered with MMCC. Your initial registration is valid for two years or until your Maryland Board of Physicians license
expires, whichever comes first.

The requirements for issuing a written certification for medical cannabis are:


  • A “bona fide physician-patient relationship” with the patient
  • The patient’s condition must be severe
  • Other medical treatments must have proven ineffective for the patient’s condition, and
  • The symptoms must reasonably be expected to be relieved by the use of medical cannabis.

Maryland law defines a “bona fide physician-patient relationship” as a treatment or counseling relationship between a Provider and patient in which the physician reviews the patient’s relevant medical records, completes an in person assessment of the patient’s medical history and current medical condition, creates and maintains medically standardized records, expects to monitor patient program and takes any medically indicated action to follow up.

Once issued, a Provider may amend or revoke a patient’s written certification on any medical grounds or if the patient no longer meets the Provider’s inclusion criteria, or the patient now meets the Provider’s exclusion criteria. Examples of exclusion criteria include suspicion on the part of the physician that the patient is abusing cannabis or diverting cannabis to others.
